Annual Recertification Appointments - If you have an annual recertification appointment, it will be rescheduled at a later date. Please complete, sign and date the forms in the recertification packet that was mailed to you. Mail the completed packet with the requested documents to your Housing Specialist. All family members 18 years or older must sign all forms in the recertification packet. You will receive a letter from your Housing Specialist with a new appointment date. Interim Changes - Changes in income and/or family composition must be reported in writing within ten (10) days of occurrence. Please mail or email the interim change information and supporting documentation to your Housing Specialist within ten (10) days of occurrence. The Change of Income form and other forms are available on our website, https://www.eastpointha.org/contact-us/ or it can be mailed to you. Please contact your Housing Specialist if you need a form mailed to you.
